Attention to All New Patients
Please follow these instructions on the day of your visit to our clinic:
- Arrive 10 minutes early to your appointment for paperwork
- Bring insurance card and primary physician referrals (if applicable)
- Please refrain from using perfume or cologne as this may trigger allergic symptoms in other patients
- If allergy testing is planned or possible please discontinue all allergy medications as explained on the first page of the New Patient Packet
